Secondary 1 students
This checklist is a helpful reminder for Secondary 1 students when submitting an application to QAA by the October 29, 2010 deadline. All of the following are required when applying:
QAA Admissions Application form
Student's Personal Statement and Parent's Personal Statement forms
Certified copy of birth certificate
- Showing full names of both parents
- Signed by a legal authority

Eligibility certificate for English instruction
- Déclaration d’admissibilité à l’enseignement en anglais issued by the Provincial government
Report cards
- Last year’s June report card
- Most recent report card available from the current academic year
Application fee of $75.00
Reference letter from a current educator
- QAIS Confidential School Report or
- Letter of recommendation requested from applicant's teacher
Photo of student applicant
Entrance exam registration
- English Language Arts and Mathematics covered
Interview attendance
- Upon receipt of all required documents

Secondary 2, 3, 4 and 5 students
This checklist is a helpful reminder for Secondary 2, 3, 4, and 5 students when submitting an application by the October 29, 2010 deadline. All of the following are required when applying:
- QAA Admissions Application Form.
- Most recent report card
- June report card of the previous year
- Attendance of the student’s family at an interview

QAA acceptance
- Admission decisions are made after a thorough examination of all information provided.
- Admission decision letters are mailed on or after the 30th of November.
